Appropriate Level of Care

Lacy Clayton Wellness operates as an outpatient clinic for medication management services during established business hours only. There is no on call coverage. ​If your medical history requires a higher level of care, the provider can help refer you to one of the many local agencies in the area that supports that level of care. Your safety and well-being are the highest priority, and we want to ensure that you are met with the appropriate level of support and various treatment modalities necessary for optimal treatment for you.

patient portal

All active patients are required to have a patient portal. To ensure patient privacy and efficiency of care, this is the primary – and preferred – method of communication. It is the expectation that patients routinely check and respond to important messages sent via the portal. Refill requests and other treatment questions should be sent via the portal. Appointment scheduling, rescheduling, and cancellations should be made by the patient via the self scheduling option in the patient portal.

paperwork completion

All active clients must complete the required consent forms, patient screeners, and other information requested in the patient portal. A valid credit card on file is required. Patients are responsible for maintaining updated insurance information in the patient portal. Appointments are subject to cancellation if there are lapses in required information.

Cancellations/No Shows

Please remember to cancel or reschedule 24 hours in advance. You will be responsible for the fee of $100 if cancellation is less than 24 hours. This is necessary because a time commitment is made to you and is held exclusively for you. Please note telephone attempts to connect are NOT made. It is the patient responsibility to reach out during the first 5 minutes of their appointment time if they are having difficulty connecting. If you do not show by 15 minutes from the start of the appointment, it will be considered a no-show.

Medication refill requests

If you have less than a week medication supply without an upcoming appointment, first contact your pharmacy to ensure remaining refills are not on file. Most often there are refills that the pharmacy simply needs to activate. Otherwise, please contact the office via portal message. Please allow up to 2 business days for the approval. We do not respond to automatically generated pharmacy requests. Typically, your refills will be sent during your follow up appointment to last until your scheduled next visit without a need to refill in between appointments.

communication

If you need to contact the provider between sessions, the preferred method of communication for established patients is through the patient portal messaging or email. Most inquiries will be responded to within the same business day, but please allow up to 48 business hours for a response. For any medication changes, an appointment is required. If a true emergency situation arises, please call 911 or go to the nearest local emergency room.

benzodiazepine policy

There is vast literature available detailing possible effects of long-term benzodiazepine use. This class of medication is indicated for short-term management of anxiety only. Long-term use of this class of medications may lead to rebound anxiety, poor sleep and insomnia, increased tolerance, physical dependence, and possible memory changes. For these reasons, benzodiazepines are prescribed sparingly in this practice.

Stimulant Policy

Any request for stimulants will need to be accompanied by documentation of confirmed ADHD diagnosis. If you do not have a formal evaluation, the provider can provide referrals for neuropsychological testing that is a comprehensive measure to evaluate for ADHD. Non-stimulant medication options can be discussed until formal evaluation is completed. 

termination

Termination of services may occur at any point by the patient if you deem that your needs are not met at this practice. Please notify us, if possible, if you plan to transition to another provider. From the provider’s end, termination may occur in the following circumstances:

  1. In the event of misuse of prescriptions
  2. Non-payment
  3. 3 or more no shows or late cancellations with less than 24 hour notice
  4. Inactivity for over 6 months without any follow up
form completion

Any request for forms or letters must be requested in advance and will be completed during an appointment. Depending on the request, the provider will let you know if the form can be completed. A $25 charge will be applied to any forms completed outside of a scheduled appointment. Please allow up to two weeks for any forms to be completed. This practice does not complete any disability paperwork.
